Article Type: Review Article (Compilation)

Abstract

Background: Breathing holds an important place in many traditions of yoga and meditation. Among breathing-related methods, pranayama primarily emphasizes the conscious regulation and direction of breathing, whereas Ānāpānasati is based on observing and being aware of inhalation and exhalation. Despite sharing an attention to breathing, these two approaches are not identical in terms of their purpose or the manner in which they intervene in the breathing process.

Objective: This article aims to review and compare the general principles, practice methods, available research evidence, and safety considerations associated with pranayama and Ānāpānasati.

Methods: This article is a narrative, nonsystematic review. Educational and book sources were used to describe traditional methods, and review articles and experimental studies related to slow breathing and selected pranayama techniques were examined to assess the research evidence. A systematic assessment of risk of bias and a meta-analysis were not performed in this article.

Findings: Available evidence indicates that slow breathing may be associated with changes in autonomic nervous system activity and in some cardiovascular and neural indices. However, evidence concerning specific pranayama techniques is more limited and heterogeneous. Some experimental studies have also reported different responses to techniques such as Ujjayi, alternate-nostril breathing, and Kapalabhati. In contrast, for the sixteen-step model of Ānāpānasati in Buddhadasa’s interpretation, sufficient direct experimental evidence to evaluate its clinical effectiveness was not found in the sources reviewed. There are also reports of adverse events associated with some intensive yoga and breathing practices, indicating the importance of gradual progression and attention to individual conditions.

Conclusion: Pranayama and Ānāpānasati can be regarded as two distinct but related approaches to attention to breathing. In introductory instruction, calm attention to the breath and simple practices without breath retention or excessive force may provide a cautious starting point. The selection and intensity of practice should be appropriate to the individual’s goal, experience, and physical condition, and extrapolation of limited study findings to all individuals or all breathing techniques should be avoided.

Keywords: Pranayama, Ānāpānasati, slow breathing, Ujjayi, mindfulness, yoga therapy

Educational Note: The contents of this article are not a substitute for medical diagnosis or treatment. In individuals with illness or specific conditions, the selection and intensity of breathing practice should be determined according to the individual’s condition.

Introduction

Breathing is one of the body’s fundamental processes and, at the same time, one of the few physiological activities that can also be regulated consciously. This characteristic has led to an emphasis on breathing in various traditions of yoga, meditation, and some contemporary methods of stress regulation.

Pranayama is a set of methods for regulating and directing the breath in the traditions of yoga. Depending on the method, these practices may include changing the speed and depth of breathing, alternate-nostril breathing, producing a breathing sound, or, in some methods, breath retention. Therefore, pranayama cannot simply be equated with “deep breathing.”

Ānāpānasati is a Pali term that generally refers to “mindfulness of inhalation and exhalation” or “awareness of breathing.” In the Buddhist tradition, this practice is based on awareness and observation of the breath and, in Buddhadasa’s interpretation, is explained in the form of sixteen steps organized into four sets of four steps.

An important difference between these two approaches lies in the role of breathing. In many pranayama methods, the individual intentionally changes characteristics of the breath, whereas in Ānāpānasati, observation and awareness of the breathing process are the central focus of the practice.

Therefore, although both approaches use breathing as the focus of practice, they should not be regarded as two names for a single method.

Method of Source Compilation

This article is a narrative, nonsystematic review developed with the aim of providing an educational and comparative overview of two breathing approaches.

The sources used fell into three broad groups:

Traditional and educational sources: To explain concepts and methods of practice.

Scientific review articles: To examine the overall state of evidence concerning slow breathing and pranayama.

Experimental studies: To examine reported outcomes concerning specific techniques.

In selecting research sources, attention was paid to the presence of the article in scientific databases, bibliographic details, and its relevance to the subject of the article.

This article is not a systematic review; therefore, the search was not conducted for all sources using a registered and reproducible protocol, and a formal assessment of study risk of bias was not performed.

Traditional and educational sources were also used to describe practice methods and the traditional perspective and were not considered evidence of clinical effectiveness.

Pranayama and Ānāpānasati

Pranayama

Pranayama can be regarded as a set of methods for the conscious regulation and direction of breathing in the traditions of yoga. Different pranayama methods vary in terms of speed, depth, breathing pathway, sound production, and the presence or absence of breath retention.

Some methods are calm and slow, whereas others involve active and rapid exhalations or specific breathing patterns.

Therefore, when scientifically examining pranayama, each technique should, as far as possible, be examined separately, and the results of one technique cannot be generalized to all types of pranayama.

Ānāpānasati

Ānāpānasati means mindfulness of inhalation and exhalation. In Buddhadasa’s interpretation, this practice is described in the form of sixteen steps organized into four sets of four steps.

In the early stages, attention to the body and the breathing process is important, while in later stages attention is directed toward feelings, the mind and mental processes, and ultimately deeper aspects of observation and insight.

Consequently, Ānāpānasati should not be regarded merely as a breathing technique for changing the speed or depth of breathing; rather, within this framework, breathing is an object for cultivating attention and mindful observation.

Preparation for Practice

In the educational sources reviewed, a quiet environment, a comfortable seated position, a relatively upright spine, and attention to bodily comfort are considered principles of preparation.

Practice when the individual is very full is generally not recommended. It is also preferable to practice in an adequately ventilated environment and under conditions in which the individual can focus on breathing without disturbance.

For beginners, sitting comfortably on a chair or bench may also be appropriate, and practice does not necessarily depend on difficult seated postures.

The important principle in introductory practice is to maintain comfort and avoid unnecessary force on the breathing process.

Calm and Mindful Breathing

One of the simplest ways to begin practice is to attend to inhalation and exhalation without attempting to change them substantially.

In this method, the individual may choose the movement of the abdomen, the chest, or the flow of air through the nose as the object of attention.

If breathing naturally becomes slower, it can be followed without forcing it. The aim of this stage is not to create intense control over breathing, but to establish stable and calm awareness of the breathing process.

Conceptually, this method is more consistent with the observational aspect of Ānāpānasati.

Important Pranayama Techniques

Ujjayi

Ujjayi is a form of yogic breathing that is generally practiced by producing a gentle sound in the throat and relatively controlling the airflow.

In a small experimental study, Ujjayi, compared with some other methods, was associated with changes in subjective measures such as calmness and stress.

However, the result of a small study should not be regarded as proof of a definitive effect of Ujjayi for all individuals.

Nadi Shodhana

Nadi Shodhana, or alternate-nostril breathing, involves alternating the breathing pathway between the two nostrils.

Some studies of this method have reported changes in physiological and cognitive measures, but the body of evidence remains limited.

Bhramari

In Bhramari, exhalation is usually accompanied by a gentle humming-like sound.

A systematic review specifically addressing Bhramari exists, but the studies included in it had methodological limitations. Therefore, broad therapeutic claims about this technique should be expressed cautiously.

Shitali and Sitkari

These methods use specific patterns for the intake of air and are introduced in traditional sources for various purposes.

However, many specific traditional claims concerning the therapeutic effects of these methods were not directly confirmed in the research studies reviewed in this article.

Kapalabhati

Kapalabhati consists of active and relatively rapid exhalations and differs in intensity from calm and observational breathing.

In a small study, this technique was associated with an increase in some subjective measures such as vitality; however, such a finding does not mean that its general therapeutic effect has been established.

Bhastrika

Bhastrika is also considered a more active breathing technique and, because of its greater intensity, is better not practiced by beginners without appropriate instruction and gradual progression.

Research Evidence on Slow Breathing

One area with more research evidence than many specific pranayama techniques is slow breathing.

A systematic review in 2018 examined studies of slow breathing, usually fewer than 10 breaths per minute, in healthy individuals.

The findings of this review indicated that slow breathing may be associated with changes in autonomic nervous system activity, heart rate variability, and some measures of brain activity.

However, these findings concern “slow breathing” across a set of different protocols and cannot be generalized without specific examination to all pranayama techniques or simply to abdominal breathing.

Research Evidence on Yogic Breath Regulation

A scientific narrative review of yogic breath regulation examined studies on pranayama and yogic breathing from PubMed, PubMed Central, and IndMed.

This review summarized reported work in physiological, psychophysiological, respiratory, and cognitive domains and in some clinical conditions.

However, the heterogeneity of pranayama methods and differences among study designs mean that the same outcome cannot be assumed for all techniques.

Study of Four Breathing Techniques

In an experimental study, four yogic breathing methods, including Ujjayi, paced breathing, Kapalabhati, and alternate-nostril breathing, were examined.

The study included 36 participants and an eight-week program.

The results indicated that participants’ responses to different techniques were not identical. Ujjayi was associated with some measures of calmness and reduced stress, rhythmic breathing with increases in some measures of interoceptive awareness, and Kapalabhati with increases in some measures of vitality.

This study indicates that even among different pranayama techniques, a single effect cannot be assumed for all methods.

Alternate-Nostril Breathing

In a study of 15 healthy men, alternate-nostril breathing was compared with two other conditions, namely breath awareness and quiet sitting.

In that study, alternate-nostril breathing was associated with a reduction in systolic blood pressure and a change in performance on a vigilance test.

Given the small sample size and the healthy status of the participants, this finding cannot be directly generalized to individuals with illness, women, older adults, or the long-term effects of the practice.

Ānāpānasati and Research Evidence

In this article, Ānāpānasati is explained based on Buddhadasa’s interpretation of the sixteen-step practice.

Buddhadasa’s book is an important traditional and interpretive source for explaining this framework, but its nature differs from that of a clinical study or controlled trial.

In the search for the sources used for this article, sufficient direct experimental evidence to evaluate the clinical effects of the complete sixteen-step model of Ānāpānasati was not found.

Therefore, at present it is more appropriate to introduce Ānāpānasati in the article as a traditional mindfulness method based on observation of breathing rather than as a therapeutic method with established clinical effectiveness.

Safety Considerations

Breathing practices are not uniform in intensity or method of execution.

Calm, effortless breathing differs considerably from techniques involving rapid, active exhalations or prolonged breath retention.

In a systematic review of case reports of adverse events associated with yoga, 35 case reports and 2 case series, comprising a total of 76 cases, were examined. Among the reported cases, pranayama and intensive breathing were also represented.

However, the case-report nature of this evidence does not allow the incidence of adverse events or their relative risk among all practitioners to be determined.

Therefore, the appropriate conclusion is not that “pranayama is dangerous,” but rather that intensive practices should be performed with caution, appropriate instruction, and gradual progression.

Individuals with cardiovascular, respiratory, or other specific medical conditions should select the intensity and type of practice according to their condition and, when necessary, seek guidance from a health professional.

Breath Retention and Straining

Some forms of pranayama include breath retention or greater force applied to the respiratory cycle.

Expiratory straining against a closed airway can be physiologically similar to the Valsalva maneuver. Therefore, exhalation or breath retention should not be performed forcefully in order to increase the intensity of practice.

For beginners, practices without prolonged breath retention and without excessive force may provide a more cautious starting point.

Suggestion for a Gradual Practice Process

Stage One

Calm breathing and attention to inhalation and exhalation for approximately 5 minutes, without breath retention and without attempting to substantially increase the depth of breathing.

Stage Two

After adaptation, simpler techniques such as gentle Ujjayi or Nadi Shodhana may be added with appropriate instruction.

Stage Three

More active techniques such as Kapalabhati or Bhastrika are better introduced after familiarity with breathing principles and at an intensity appropriate to the individual.

Alongside these stages, mindful observation of breathing can be practiced as an independent mindfulness exercise.

This progression is an educational suggestion by the compiler of the article and is not a validated therapeutic protocol.

Discussion

The review of the sources indicates that both pranayama and Ānāpānasati use breathing as the focus of practice, but the role of breathing in them is not the same.

In pranayama, depending on the technique, breathing may be the subject of regulation, control, and intentional modification. In Ānāpānasati, attention to and observation of the breathing process are central.

Regarding research evidence, slow breathing has been studied more extensively than many specific pranayama techniques. At the same time, studies specifically examining different techniques have not shown uniform results.

This highlights the importance of examining each technique separately.

It is also necessary to distinguish among traditional claims, research findings, and the author’s educational suggestions. A claim presented in a traditional or educational book cannot, merely because it appears in that source, be presented as an established therapeutic effect.

Limitations of the Article

This article has limitations that should be considered when interpreting the findings:

The article is a narrative and nonsystematic review.

The literature search was not conducted using a registered and reproducible protocol for all sources.

A formal assessment of study risk of bias was not performed.

The quality and type of evidence varied among studies.

Some sources used were traditional or educational.

Direct evidence concerning the sixteen-step model of Ānāpānasati is limited.

Some pranayama studies had small samples.

Findings concerning slow breathing cannot be generalized to all pranayama techniques without specific examination.

Conclusion

Both pranayama and Ānāpānasati use breathing as the focus of practice, but they differ fundamentally in their purpose and in how they approach the breathing process.

Available research evidence, particularly concerning slow breathing, supports the existence of some psychophysiological effects; however, specific evidence concerning many pranayama techniques remains limited, heterogeneous, or based on small studies.

Regarding the sixteen-step model of Ānāpānasati in Buddhadasa’s interpretation, sufficient direct experimental research to draw conclusions about its clinical effectiveness was not found in the sources reviewed.

From an educational perspective, beginning with calm attention to breathing and simple practices without force or prolonged breath retention may be a cautious approach. More active techniques should be taught gradually and in accordance with the individual’s experience and condition.

Larger, controlled studies with more rigorous methodologies are needed to translate traditional claims about breathing into therapeutic recommendations.
